What Do Our Social Workers Do?

  • A case manager’s primary goal is to provide support, resources, and community services needed for seniors to safely remain in their own homes
  • Case managers conduct home visits to assess individual needs, and will provide information, referrals and advocacy according to the needs
  • Services include but are not limited to:
    • Housekeeping
    • Laundry
    • Lawn care/snow removal
    • Minor home repairs
    • Assistance with health and prescription insurance
    • Assessment of eligibility for federal, state and county programs
    • Caregiver information/support groups
    • Elder abuse and neglect
    • Financial/utility assistance programs
    • Recreational/social opportunities
  • Case managers provide both short-term assistance as well as ongoing
    case management support
  • Case management services are provided free of charge, although donations to the senior center are always gladly accepted
